Insurance Information
We participate with many commercial insurance plans, as well as Medicaid and Medicare. We bill insurance as a courtesy but cannot guarantee coverage. Before your first visit, we will ask which insurance plan(s) you would like us to bill and will verify coverage to the best of our ability. You are responsible for any deductibles, co-pays, or co-insurance, which are due at the time of service. If we are out of network, we can check whether exceptions apply.

However, we also work with additional secondary, supplemental, and auto insurance plans (including Medicare secondary policies such as UMR and others). Coverage varies by plan, so we encourage you to contact our office and we will verify your benefits before your first visit.
